Van Gogh probably painted this grisly little picture while at the art academy in Antwerp. Skeletons were often used during drawing lessons for the study of human anatomy. Van Gogh undoubtedly intended the cigarette as a joke, perhaps even as a critique of the rigid and pedantic method of academic training. He stayed at the school for only a few weeks.
